Accessioning Tissue Specimens
Procedure
- Match corresponding requisition to specimen making sure the name, date of birth and health card number are on both the requisition and specimen*.
- Record Patient information into next available spot in the LOG book. Separate like samples (example: do not put two small bowels back to back in log book) and similar names (example: Johnson and Johnston)
- The accession number will be the next consecutive number in the LOG book. Record patients full name, Health card number, specimen type, date and time of collection, as well as date and time received. Provide your initial in the space provided.
- Record the accession number on both the requisition and the specimen.
*NOTE If there are discrepancies between requisitions and specimens, an incident report form (QA form) must be filled out as a quality measure. For our purposes we will attach the incident report form to the requisition, the instructor will advise the resolution to the issue, and the accessioning process will proceed. The QA form MUST accompany the requisition when submitting.
* If any fixative was added, that is to be recorded on the original requisition.
